Fit Note
If you have been ill for 7 days or less, you do not need a fit note. You can self-certify instead. Many employers have their own self-certification form. If yours does not, use the self-certification form on GOV.UK: print it, fill it in and give it to your employer.
Some employers ask for a doctor’s note even for a short absence. This is a private service and costs £40.
Illness lasting more than 7 days
If you have been ill for more than 7 days, your employer will usually ask for a fit note. Its official name is a Statement of Fitness for Work.
A clinician may call you or arrange an appointment before issuing it.
If you have already been seen about this illness, your fit note will be sent to you within a week.
