We have made a change to our prescription ordering system. The new system is similar to old form and self explanatory but you will need to delete any bookmarks you have to the old form as it will no longer be available. Any details you have saved will however be lost and you do need to register for the new system. You can do this via the link above. Please note: online accounts will have limited access until the patient has confirmed their identity by bringing a photographic ID into the surgery.
Please hand in, post or fax a written prescription allowing 48 working hours for the prescription to be processed.
Please do not let your medicines run out as it is not possible to process it while you wait.
No prescription requests are taken over the telephone.
Patients taking repeat medication should be checked at regular intervals, please ask your doctor how often you need to be seen. Prescriptions can be collected by one of the local chemists if you indicate this on the form.

Patients on repeat medication will be asked to see a doctor at least once a year to review these regular medications and notification should appear on your repeat slip.
Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
