New Patient Registration

We are pleased to welcome new patients who live within the practice area (click here to see a map).

If you live in our practice area and would like to register with us, please do so by completing the necessary forms available from reception. Your registration will be with the practice but we organise the care of our patients by allocating patients to a specific GP who works with a personal secretary. For continuity of care we encourage you to always consult with your own GP if they are available. We ask all new patients to complete a medical questionnaire. Please answer all the questions for our records.

We are fully registered under the Data Protection Act. Patients' records are only available to doctors and staff in the practice. We recognise the need for confidentiality and discretion at all times.

Change of Personal Details

If you change your name, address or telephone number, please contact your doctor's personal secretary. If you move out of the practice area, please do not assume that we can continue to care for you.
